Periodontal Maintenance

Periodontal maintenance is the cleaning that follows gum treatment, and it happens every three to four months. Bacteria rebuild inside a deep pocket in about three months, so a six month interval lets the infection restart. It is how the result you paid for is kept.

Why every three to four months?

Because the bacteria come back on a schedule. In a treated pocket the bacterial population returns toward its original level in roughly nine to twelve weeks. Cleaning at that point resets the count before the tissue breaks down again.

Six month cleanings are built for people who have never had bone loss. Once you have had periodontitis, the pockets are permanent architecture. They collect plaque in places a brush cannot reach, however carefully you clean at home. Whether your interval is three months or four is set by your numbers, your smoking history and your medical conditions.

How is it different from a regular cleaning?

A regular cleaning removes deposits from tooth surfaces above and just below the gum line in a healthy mouth. Maintenance cleans inside pockets that are already deeper than normal, on root surfaces rather than enamel, and it takes longer.

The visit also includes charting every time, not once a year. We use the same six measurements per tooth so the comparison is fair. Local anesthetic is available for tender areas. What is measured at each visit?

Five things are recorded. Pocket depths at six points per tooth. Bleeding on probing, which is the earliest warning. Recession, so we can see whether the gum line is moving. Mobility of each tooth. And bone loss between the roots of a molar.

We also check old fillings and crown margins, since a rough edge holds plaque, and we look for new decay on exposed roots. X-rays are taken based on risk, usually bitewings every one to two years. Your numbers are compared against your last visit and printed for you to keep.

What happens if pockets come back?

If pockets deepen again, we find out why before adding treatment. Common reasons are a missed area in daily cleaning, a new crown margin that traps plaque, a change in medication, poorly controlled diabetes, grinding or continued smoking.

The response is matched to the cause. A single site may need re-cleaning under local anesthetic or a local antibiotic placed in the pocket. Several sites may mean repeating scaling and root planing in that area. Sites that stay at five millimeters or deeper are reviewed with the visiting periodontist who comes to Suite 450.

What maintenance will not do

Maintenance will not rebuild the bone you lost. It holds the ground you have. That is a smaller promise than people want, and it is the honest one, so we track bone levels on x-rays taken a year or two apart to show whether the ground is holding.

It will not replace daily cleaning either. Three or four visits a year add up to about two hours. You cover the rest of the year. We spend part of each visit on technique with interdental brushes and, where useful, a water flosser, because that is where most of the result is decided.

What does periodontal maintenance cost?

Maintenance is quoted in writing, itemized, after an examination, and it is charged per visit. We tell you the yearly figure at the start, so there are no surprises in month nine.

We are in network with most major PPO plans and Delta Dental HMO. Plans differ on frequency: some cover two cleanings a year, some cover three or four when the chart supports it, and some alternate maintenance with a regular cleaning. We check your specific limits and tell you what falls outside them before the appointment. Third party financing is available and the practice is not the creditor. Our membership plan is an option without insurance.

Questions people ask about periodontal Maintenance

Why can I not go back to two cleanings a year?

Some people can, after several years of stable numbers and no bleeding. It is a decision made from the chart, not the calendar. Once pockets stay at three millimeters or less with no bleeding for a sustained period, we discuss lengthening the interval. If the numbers slip at that longer interval, we move back to three or four months.

Does my insurance cover four visits a year?

Plans vary, and we check yours before you book. Some cover four maintenance visits a year, some cover two, and some alternate one maintenance visit with one regular cleaning. Benefits also reset on a plan year that may not match the calendar. We give you the covered and uncovered portions in writing so you can decide about any extra visits.

Is periodontal maintenance forever?

For most people with a history of periodontitis, yes. The condition is controlled rather than cured, and the pockets that remain collect plaque where a brush cannot reach. Think of it the way you think of blood pressure medication. The result depends on continuing, and stopping usually returns the numbers to where they started within a year or two.

Can I have maintenance somewhere else and treatment here?

You can, and many people split care for convenience or insurance reasons. It works when the other office sends us the full chart, including six point pocket depths and bleeding points, at each visit. Without those numbers we cannot tell whether anything is changing. Bring your records to your first visit in Suite 450 and we will build from them.

Do I still need x-rays at these visits?

Not at every visit. Bitewings are usually taken every one to two years based on your risk of decay, and single tooth images are taken when something specific changes. Digital x-rays use a lower dose than film and appear on the screen immediately. We take them when they will change what we do, and we tell you why beforehand.
