All-on-4 and Full Arch Dental Implants in Laguna Hills

All-on-4 is a full arch of fixed teeth carried on four to six implants, replacing an upper or lower jaw of failing or missing teeth. The bridge is screwed down, so you do not take it out. Who a full arch bridge actually helps

A full arch bridge replaces every tooth in one jaw at once. Four to six implants go into the bone, multi-unit abutments sit on top, and one solid bridge screws onto them. Only a dentist takes it off. The back implants are usually tilted forward, which uses the denser bone toward the front of the jaw and stays clear of the sinus and the nerve.

It suits people whose whole arch is going: severe gum disease with teeth that move, old crowns and roots that keep breaking, or a lower denture that will not stay put. If only a few teeth are failing, single implants or a smaller bridge fits better, which we cover on our dental implants page. If teeth across both jaws are worn and broken, look at full mouth reconstruction.

It is a poorer fit if you smoke heavily and will not stop around surgery, if diabetes is not controlled, if you take intravenous bone medicines such as bisphosphonates, if you have had radiotherapy to the jaws, or if daily cleaning is not realistic. None of these is an automatic no. They change the plan.

If you have been told there is not enough bone

Bone shrinks after teeth have been missing or loose for years, so most people considering a full arch have lost some. That is what tilting the back implants is for. It often works in a jaw that would not take four upright implants. Where it is still not enough, the options are grafting, a sinus lift, or a removable implant retained overdenture on fewer implants. The scan settles which applies.

How it works at our Laguna Hills office

1. Consultation and records

You meet one of our dentists, we examine what is left, and we take a 3D cone beam CT scan showing bone height and width, the sinus floors and the lower nerve. We also take photographs and a digital intraoral scan, which most people find easier than a tray of impression material.

2. Planning and the surgical guide

The scans are merged and the case is planned on screen before anything happens in your mouth: how many implants, where each goes, and where the teeth will sit. That plan becomes a surgical guide, a template that seats over the jaw and directs each drill. Guided placement lands close to the plan, not exactly on it: a 2024 systematic review reported average deviations of 1.11 mm at the implant head, 1.40 mm at the tip and 3.51 degrees.

3. The day of surgery

Expect most of a day, usually three to six hours. You are numb with local anesthetic throughout, and sedation is available. The remaining teeth come out, the ridge is smoothed so the bridge seats flat, the implants go in through the guide, and the abutments are fitted. A fixed temporary bridge goes on the same day in most cases. If an implant is not stable enough to load, you wear a temporary denture while it heals.

4. Healing with the temporary bridge

Swelling and bruising usually peak at two to three days and settle over the following week. You are on a soft diet for roughly six to ten weeks. We see you the next day, then at intervals while the implants bond to the bone over three to six months. The temporary bridge is acrylic and is not the finished result.

5. The final bridge

Once the implants have integrated we take final records and check the fit of the framework. You see a try-in of tooth position, shade and lip support. The final bridge is usually zirconia, sometimes titanium with acrylic teeth, and a dental laboratory makes it: our in-house CAD/CAM mill is used for same-day single crowns, not full arch bridges. Final records may be a digital scan, conventional impressions, or both with a verification jig.

6. Maintenance

You clean under the bridge daily with a water flosser and superfloss, and we show you how. Hygiene visits are usually every three to six months, and the bridge is unscrewed and cleaned at intervals. A night guard is needed if you grind.

Cost

Full arch treatment is quoted per arch, and again for both arches together. The scan and the plan come first, then an itemized written estimate, before treatment begins. We do not quote a full arch over the phone, because the figure depends on your bone and the number of implants.

The arch fee normally includes the consultation, the cone beam scan, planning and the surgical guide, the extractions in that arch, the implants and abutments, the temporary bridge, the reviews and the final bridge. It excludes the opposite arch, grafting or a sinus lift, sedation beyond local anesthetic, a night guard, and later repair or replacement. Each of those is quoted on its own line.

We accept most major PPO plans and Delta Dental HMO. Most plans contribute to the extractions and sometimes part of the implant work, and nearly all carry an annual maximum well below the cost of an arch, so we submit a pre-treatment estimate first. Timeline

From first consultation to finished bridge is usually 20 to 40 weeks, about five to nine months.

• Consultation, scan and planning: one to two visits across one to three weeks.
• Surgery, with a fixed temporary bridge the same day in most cases: one day.
• Healing while the implants integrate: usually three to six months.
• Final records, try-in and fitting the final bridge: usually four to eight weeks.

Some cases take longer. Grafting, a sinus lift, or an implant that does not integrate first time will add months.

Questions people ask about All-on-4

How much do All-on-4 implants cost in Orange County?

We quote per arch, itemized and in writing before anything begins. What moves the number is how many implants your jaw needs, whether bone work is involved, and the material of the final bridge. When comparing quotes, check what each one covers.

Is All-on-4 really teeth in a day?

You usually go home on the day of surgery with a fixed temporary bridge, so you are not without teeth at any stage. That temporary is acrylic and holds the tooth position while the implants heal. The final zirconia bridge follows three to six months later.

Does it hurt?

You are numb with local anesthetic for the whole procedure and sedation is available. Most people are comfortable during surgery. Afterwards it is soreness and swelling more than sharp pain. Swelling is at its worst at two to three days, then settles over the next week.

I was told I do not have enough bone. Can I still have this?

Often yes. Tilting the back implants forward uses the denser bone toward the front of the jaw and avoids the sinus and the nerve, which is why the technique exists. Where that is not enough, grafting, a sinus lift, or an overdenture on fewer implants are the alternatives to discuss.

How long do full arch implants last?

No dentist can promise a lifespan. Implants that integrate and are cleaned daily often stay in service for many years. The bridge on top wears and can chip, so it may need repair or replacement while the implants stay put. Grinding and smoking shorten that.

Can I eat normally afterwards?

Not straight away. You are on a soft diet for roughly six to ten weeks while the implants integrate, then you widen it gradually. Because the finished bridge is screwed to the implants, it does not lift or rock when you chew. Ice and bones stay off the list.

Am I too old for dental implants?

Age by itself is not a barrier, and full arch treatment is done routinely for people in their seventies and eighties. What matters is your general health, the medicines you take and whether daily cleaning is realistic. We ask about heart conditions, diabetes, blood thinners and bone medicines.

What is the difference between All-on-4 and a snap-in implant denture?

A full arch bridge is screwed to the implants and only a dentist takes it off. A snap-in overdenture clips onto two to four implants and you remove it yourself to clean it. It needs fewer implants, and it is quoted separately.
