Bad breath almost always has a findable cause in the mouth. Gum disease is the most common one, followed by a coated tongue, dry mouth, untreated decay and old restorations that trap food. At Pearl32 Dentistry and Wellness in Laguna Hills, we look for the source rather than covering the smell. Treatment follows the cause: gum treatment, tongue cleaning, saliva support or repairing teeth. A small number of cases are medical, and we will say so when the mouth is clear.
What causes bad breath?
Bacteria breaking down proteins and releasing sulfur compounds. Gum disease is the most frequent source, because deep pockets hold bacteria that thrive without oxygen. The back of the tongue is next, where a coating traps debris and the same bacteria. Dry mouth is a third cause, because saliva washes and buffers, and medication for blood pressure, allergies, depression and bladder control reduces it. Decay, a failing filling or a cracked crown can hold food where a brush cannot reach. Tonsil stones, sinus drainage and reflux account for some cases, and food and tobacco explain the rest.
How do we find the cause?
With an examination that measures things instead of guessing. We record gum pocket depths in millimeters around every tooth and note where they bleed. We look at the tongue coating and ask whether you clean it. X-rays show decay and failing restorations hidden between teeth. We ask about medications, mouth breathing, snoring, reflux, diabetes and the time of day the smell is worst. A digital scan and photographs help us track change over time. If the odor is stronger through the nose than through the mouth, the source may sit beyond the mouth.
What does treatment look like?
It follows the cause, largest first. If pockets are 5 millimeters or deeper, scaling and root planing comes first, with local anesthetic, usually over one or two visits. Gum treatment alone resolves a large share of cases. Tongue cleaning with a scraper, back to front, twice daily, clears most coatings in two to three weeks. Decay and leaking fillings are repaired so food has nowhere to sit. Dry mouth is managed with water, sugar free gum, saliva substitutes and a review of medication timing with your physician. We re-measure at three months to see what actually changed.
Why does mouthwash not fix it?
Because most rinses mask odor for 20 to 60 minutes without touching the source. Alcohol based rinses can make matters worse by drying the mouth further. Chlorhexidine rinses do reduce bacteria and are useful for short courses after gum treatment, though they stain teeth with longer use, so we monitor that. Rinses with zinc or chlorine dioxide neutralize sulfur compounds, which helps as a support rather than a cure. We will recommend a rinse where one fits. When is bad breath medical rather than dental?
When the mouth checks out clean and the smell persists. Sinus infection and post nasal drip are the most common causes outside the mouth, and the odor often comes through the nose. Tonsil stones produce a distinct smell and are visible as white specks at the back of the throat. Reflux, uncontrolled diabetes, kidney and liver conditions and certain medications each have recognizable patterns. We do not diagnose those. We write down what we found, what we ruled out and what we recommend, then refer you to your physician or to an ear, nose and throat doctor.
What does bad breath treatment cost?
It depends on the cause, so we quote it in writing after an examination, itemized. A visit that ends in a cleaning and a tongue routine looks very different from one that ends in gum treatment across four quadrants. You see the plan and the quote before anything is booked, and we do not publish prices.

Questions people ask about bad Breath Treatment
How do I know if I have bad breath?
Judging your own breath is unreliable, because you adapt to your own smell. Licking the back of your wrist, letting it dry for 10 seconds and smelling it gives a rough idea. Asking someone you trust gives a better one. At the office we assess it alongside gum measurements, tongue coating and x-rays, which turns a worry into something we can measure and follow.
Will a cleaning cure my bad breath?
Sometimes, and sometimes not. A cleaning removes deposits above and just under the gum line, which helps when gums are only mildly inflamed. If pockets are 5 millimeters or deeper, a routine cleaning cannot reach the bacteria causing the odor, and scaling and root planing is the treatment. We measure first, so you are not paying for a cleaning that was never going to be enough.
Should I scrape my tongue?
Yes, if there is a coating on the back of it. Use a scraper rather than a brush, work from back to front, and rinse it between passes. Twice a day for two to three weeks makes a visible difference. Do not scrape hard enough to make the tongue sore. If the coating returns within days every time, dry mouth or reflux is worth investigating.
Can dry mouth from my medication be treated?
The dryness can be managed, though we do not change your prescriptions. Sip water through the day, chew sugar free gum with xylitol after meals, and use a saliva substitute or gel at night. Avoid alcohol based rinses. Dry mouth raises decay risk on exposed roots, so we shorten recall intervals and apply fluoride varnish more often. Your physician may be able to adjust the drug or its timing.
How long before my breath improves?
It depends on the cause. Tongue coating improves within two to three weeks of daily scraping. Gum treatment shows its effect at the three month re-measure, once the tissue has tightened around the roots. Repairing a food trap helps within days. If nothing has changed after three months of following the plan properly, the cause is likely outside the mouth and we look further.
