MetLife PPO
With MetLife PPO you can book directly. Most plans cover exams, cleanings and routine x-rays at or near 100 percent, with fillings at a middle rate and major work such as crowns and root canals at a lower rate, up to your annual maximum. We confirm your deductible and any waiting periods before treatment.
MetLife HMO
A MetLife HMO plan pays only when Pearl32 is your assigned dental office. Call the number on your card and ask to change your office to Pearl32 Dentistry and Wellness, 24022 Calle De La Plata, Suite 450, Laguna Hills. HMO plans use a fixed copay for each procedure, and we put it in writing before treatment.
Federal employees and retirees
We are in network with MetLife Federal Dental through FEDVIP. Some federal health plans include only a small preventive dental rider, so we check your dental benefit before the visit. We are not in network with GEHA.
Check your benefits before you book
Call (949) 830-0074, Monday to Friday, 8 AM to 5 PM, with your plan name and the member ID on your card. If the plan is in a family member’s name, have the subscriber’s name and date of birth ready. We check your benefits while you are on the phone and give you a written, itemized estimate before any treatment beyond a routine visit.
