Guardian PPO
With Guardian PPO you can book directly. Preventive visits are usually covered at or near 100 percent, fillings at a middle rate, and crowns, root canals and other major work at a lower rate, up to your annual maximum. We check your deductible and waiting periods before treatment.
Guardian HMO
A Guardian HMO plan pays only when Pearl32 is your assigned dental office. Call the number on your card and ask to switch your office to Pearl32 Dentistry and Wellness, 24022 Calle De La Plata, Suite 450, Laguna Hills, or call us and we walk you through it. Copays are fixed per procedure, and we quote them in writing first.
