Delta Dental PPO and Premier
With a PPO or Premier plan you can book directly. Most Delta Dental PPO plans cover exams, cleanings and routine x-rays at or near 100 percent. Fillings are usually covered at a middle rate, and crowns, root canals and implants at a lower rate, up to your annual maximum. We check your deductible, maximum and any waiting periods before the visit.
DeltaCare USA and the SCAN HMO
Delta Dental HMO plans only pay when Pearl32 is listed as your assigned dental office. Call the member services number on your card and ask to change your office to Pearl32 Dentistry and Wellness, 24022 Calle De La Plata, Suite 450, Laguna Hills. The change usually starts on the first of the next month. HMO plans use a fixed copay for each procedure, and we quote it in writing before treatment.
