How Cigna DPPO works here
With Cigna DPPO you can book directly, with no referral. Our in-network Cigna dentist is Dr. Masih Rezaei, so tell us you have Cigna when you call and we book you with him. You then pay the in-network share your plan sets. Most plans cover exams, cleanings and routine x-rays at or near 100 percent, fillings at a middle rate, and crowns, root canals and implants at a lower rate, up to your annual maximum.
Before treatment
For anything beyond a routine visit, you get a written, itemized estimate showing what Cigna is expected to pay and what you pay. For larger treatment we can send a pre-treatment estimate to Cigna so its share is confirmed in writing before you decide.

Check your benefits before you book
Call (949) 830-0074, Monday to Friday, 8 AM to 5 PM, with your plan name and the member ID on your card. If the plan is in a family member’s name, have the subscriber’s name and date of birth ready. We check your benefits while you are on the phone and give you a written, itemized estimate before any treatment beyond a routine visit.
