All-on-4 vs Dentures: How to Choose

A full arch implant bridge, often called All-on-4, is fixed to four to six implants and stays in your mouth. A denture rests on the gum and comes out at night. The right choice depends on your bone, your health, your budget and how much a loose plate bothers you. The short answer

Choose the fixed bridge if you want teeth that do not move, you can commit to five to nine months of treatment, and your bone and health allow surgery. Choose a denture if you want no surgery, a lower cost, or a quicker result. A snap-in implant denture sits between the two, and we cover it below.

How the two actually differ

How they stay in

The All-on-4 bridge is screwed onto four to six implants set in the jawbone. Only a dentist takes it off. A conventional denture holds by suction on the upper jaw and mostly by muscle control on the lower. A lower denture on a flat ridge is the one people complain about most.

Chewing

A fixed bridge carries chewing force into the bone through the implants. After the healing diet, most people get back to a normal range of foods, with ice and bones off the list. A denture passes the force to the gum. Bite strength is lower and hard or sticky foods are harder work.

The bone underneath

Once teeth are gone, the jawbone shrinks because nothing loads it. A denture sitting on the ridge does not stop that, which is why dentures loosen over the years and need relining. Implants load the bone and slow the loss around them.

Daily care

A denture comes out to clean and soaks overnight. A fixed bridge is cleaned in the mouth with a water flosser and floss threaders under the bridge, then checked and cleaned at the office. Neither one is maintenance free.

Time and surgery

A denture is made over a few visits across several weeks and needs no surgery, though any extractions still have to heal. All-on-4 needs a surgical day under local anesthetic, with sedation available, then three to six months while the implants bond to the bone. First consultation to finished bridge is usually 20 to 40 weeks.

Where each one falls short

All-on-4 is not for everyone. Heavy smoking that will not stop around surgery, uncontrolled diabetes, intravenous bone medicines and past radiotherapy to the jaws all count against it. So does a daily cleaning routine you know you will not keep up. The temporary bridge you wear while healing is acrylic and is not the finished result.

Dentures have their own limits. An upper denture covers the palate and dulls taste and temperature. A lower denture moves when you talk and eat. Adhesive helps for a while. Relines are needed as the ridge changes, and the plate itself is usually replaced every five to ten years. None of that is failure. It is how a denture works.

The middle option: a snap-in implant denture

An implant supported denture clips onto two to four implants and you take it out yourself to clean it. It stops a lower denture floating, needs fewer implants than a fixed bridge, and costs less than one. It still comes out at night, and the clips wear and need replacing every year or two. If only a few teeth are failing rather than the whole arch, single implants or a small bridge usually fit better.

Questions people ask about All-on-4 versus dentures

Is All-on-4 better than dentures?

Neither is better for everyone. A fixed bridge wins on chewing, stability and keeping the bone. A denture wins on cost, no surgery and speed. Your bone on the cone beam scan and your general health decide which one is realistic for you.

Can I switch from dentures to All-on-4 later?

Usually, yes. Years in a denture shrink the ridge, so a graft or tilted implants may be needed to make room for the fixtures. The sooner after tooth loss you ask, the more bone there is to work with. The 3D scan shows exactly what is left.

Does All-on-4 feel like real teeth?

Closer than a denture does. Nothing covers the palate and nothing moves when you talk or eat. It is still a bridge, so you may notice some bulk near the gum for the first few weeks. Speech usually settles within two to four weeks.

How much more does All-on-4 cost than a denture?

A fixed bridge costs more than a denture because it includes surgery, the implants and a milled bridge. We quote per arch, itemized and in writing, after the scan. We do not quote a full arch over the phone. Third party financing is available and the practice is not the creditor.

Will dental insurance pay for either option?

Most dental plans contribute toward a denture. Implant coverage varies by plan and annual maximums are small next to a full arch fee. We check your plan and show you the numbers before you decide anything.

Am I too old for All-on-4?

Age by itself is not a barrier. What matters is how you heal, medications such as intravenous bone drugs, and whether you can clean under the bridge every day. A medical history and the scan decide, not a birthday.
