Digital dentistry means your mouth is recorded as data rather than film and putty. What digital dentistry covers, and when it matters

Digital dentistry is not one machine but a chain of them. Each step writes a file the next step reads, so the shape of a tooth or the path of an implant is carried forward as measurement rather than redrawn by hand at each stage.

A cone beam CT records bone in three dimensions. An intraoral scanner records tooth and gum surfaces. The two line up on each other, which is what lets an implant be planned against bone, teeth and bite at once.

It matters most when a decision is hard to undo: an implant near a nerve, a full arch plan, or veneers where the shape is agreed before enamel is touched.

It decides nothing. A scan carries more information than a flat film can, and gives you something to look at while the options are explained. Pearl32 Dentistry and Wellness has been serving the community since 1994, and these tools sit inside ordinary general dentistry, not in place of it.

The four systems, and what each one does

3D cone beam CT

A cone beam CT arm rotates once around your head and builds a three dimensional volume of your jaws. Nothing goes in your mouth, the scan takes under a minute, and the result can be sliced in any direction and measured at true size.

A standard dental X-ray flattens a three dimensional object onto one plane. It stays the right tool for checking decay between teeth, but it cannot show depth: structures overlap, and anything behind something denser is hidden. A cone beam scan shows what a film cannot.

  • Bone width, not only height. A ridge can look tall on film and be too thin to hold an implant.
  • Where the lower jaw nerve canal runs and exits, mapped before a drill goes near it.
  • How much bone sits below the sinus floor above the upper molars.
  • Root canal anatomy: extra canals and sharp curves that overlap into invisibility on film.
  • Infection, root resorption and impacted teeth in three dimensions.

The honest trade is dose. A cone beam scan uses more radiation than a standard dental X-ray, and much less than a medical CT of the head. Dose rises with the area captured, so we scan only when it will change the plan, and scan the smallest area that answers the question.

Digital intraoral scanning

An intraoral scanner is a wand with a camera on the end. It passes over your teeth while software stitches the images into a 3D model you watch appear on screen.

What you do not have is a tray of material setting in your mouth. If one area comes out poorly we rescan it rather than starting the whole impression again, and the file goes straight to the mill here or to a laboratory.

We do not claim scanning is more accurate than a conventional impression. Cai and colleagues, comparing the two methods for full arch implant work in 2024, reported that conventional impressions performed better where implants were parallel. Conventional materials are still used here where a case calls for them. Digital scanning is faster and more comfortable, which is the claim we do make.

Guided implant surgery

Guided surgery means the implant is positioned on the computer first, then placed through a guide that reproduces that position in your mouth. We decide where the crown has to sit, then work back to the implant that supports it.

  1. The cone beam scan and the intraoral scan are merged into one file.
  2. The implant is planned on screen: position, angle, depth, length and diameter, with clearance from the nerve, the sinus and the neighboring roots.
  3. A guide is made to seat on your teeth or ridge, with sleeves holding the drills on that path.
  4. On the day, the drills run through the guide, so the plan is followed rather than improvised.

A guide narrows the gap between plan and placement. It does not close it. Khaohoen and colleagues, reviewing guided placement in BMC Oral Health in 2024, reported mean deviations of 1.11 mm at the implant head, 1.40 mm at the tip and 3.51 degrees of angle. We plan with that margin in mind, and leave clearance rather than using every millimetre of bone.

In-house CAD/CAM milling

CAD/CAM means computer aided design and computer aided manufacture. After a tooth is prepared we scan it, design the restoration on screen against the neighboring and opposing teeth, then cut it from a ceramic block in a mill here, usually ten to twenty minutes for a single unit. It is fired in a small furnace, adjusted and bonded the same day.

For a suitable crown, inlay or onlay that is one appointment instead of two, with no temporary to come loose in between. It does not suit every case: several front teeth together, shades layered by hand, long span bridges and most full arch work go to a laboratory technician. We say which route your case is on first. What this means for you, in plain terms

  • Fewer appointments for some treatment. A milled crown can be finished in one visit: one lot of anesthetic, no temporary.
  • Nothing setting in your mouth. Scanning has replaced trays for most records, which matters if you gag.
  • You can see your own mouth. The screen turns toward you: the crack, the bone level, the shadow at a root tip. It is harder to feel talked into treatment you have looked at yourself.
  • Surgery planned in advance rather than judged on the day. Angle, depth and whether a graft is needed are decided at a desk, not in the chair with the clock running.

None of this makes treatment risk free, or removes the need for judgement and daily cleaning. It changes the information, not the dentistry.

What it costs

Digital dentistry is not something you buy on its own. The scan, the planning and the milling sit inside the treatment they support, and each is a line on your estimate: the cone beam scan with 3D planning, and the digital records. How long the digital steps take

How long depends on the treatment. The digital steps are rarely the slow part.

  • Cone beam scan: a few minutes, and we look at it with you the same visit.
  • Intraoral scan of both arches and your bite: usually five to fifteen minutes.
  • Milled crown: usually one appointment of two to three hours.
  • Guided implant surgery: usually two to four weeks from planning scans to surgery date, then three to six months of healing before the final tooth.

Ranges, not promises. Complex cases take longer, and we will add a week rather than rush a step.

What does a digital dentist actually do differently?

How much radiation is in a dental cone beam CT scan?

Do I need a referral for a cone beam CT scan?
