What clear aligners are and who they are for

An aligner is a clear plastic tray molded to your teeth, each in the series slightly out of step with where your teeth sit now. That mismatch is the point: seated, the tray presses on particular teeth in a particular direction.

Light, sustained pressure is what moves a tooth. Bone ahead of the root is resorbed and new bone fills in behind it, so the tooth travels through the jaw and the socket rebuilds around it. Braces do the same with brackets and wires; the difference is that trays come out.

Sold as invisible braces, aligners suit adults with sound teeth and gums who have mild to moderate crowding, spacing, or teeth that have drifted since childhood orthodontics. They are not a treatment for disease, so decay and bleeding gums come first. The brand is the manufacturing system behind the trays, and ours is Candid Pro.

If you had braces as a teenager

This is the most common reason adults come in. Teeth straightened once and then left without a retainer drift back, and the lower front teeth almost always go first. How aligner treatment runs at our Laguna Hills office

Pearl32 Dentistry and Wellness has three general dentists, serving the community since 1994.

• 1. Examination and records. Teeth, gums and bite first, then photographs and a digital intraoral scan, which takes a few minutes and no impression material, plus a 3D cone beam CT scan where roots or bone need checking.
• 2. The plan. Your scan becomes a digital plan: movement tooth by tooth, the number of trays, where attachments go, where enamel reduction is needed. It is a projection, not a promise.
• 3. Wearing the trays. 20 to 22 hours a day, out for meals and anything other than water, changed on schedule.
• 4. Reviews. We check that teeth are tracking and gums are healthy.
• 5. Refinement. Common, and not a failure: we rescan and order a shorter series to finish what did not track.
• 6. Finishing. Attachments polished off, retainers fitted, then whitening or small bonding adjustments if you want them.

Attachments and enamel reduction, explained

Smooth plastic slides over a smooth tooth, so a tray alone can tip a tooth but struggles to rotate one, pull one down or move a root. Attachments, small tooth-colored composite bumps bonded on without drilling or anesthetic, give the tray something to grip. They are polished off at the end.

Interproximal reduction, or IPR, is enamel removal, and it is worth being blunt about that. Crowded teeth need space, which comes from an extraction, from widening the arch, or from a fraction of a millimetre taken at the contacts. Enamel does not grow back, so we do it only where the plan needs the room.

Wear time is the treatment

20 to 22 hours a day leaves two to four hours out, which sounds generous until you count breakfast, coffee, lunch and a long dinner. Compliance is the single biggest factor in whether aligner treatment works.

When wear time slips, the tray no longer seats fully, teeth fall behind the plan, and the case ends up rescanned and restarted.

What aligners do well, and what they do not

Aligners handle many adult cases: mild to moderate crowding, particularly of the lower front teeth; spaces and gaps; relapse after earlier orthodontics; and aligning teeth before cosmetic work, so veneers afterwards can be fewer and thinner.

They are limited, or the wrong tool, here:

• Severe crowding needing extractions and roots moved bodily across the arch.
• Significant bite correction. A large overjet, deep bite or skeletal difference between the jaws may need braces with elastics, or a surgical opinion.
• Some rotations. Round-sided premolars and canines give a tray little to hold.
• Teeth already crowned or veneered, because composite does not bond to ceramic as it does enamel.

Retainers, and the comparison people actually want

Teeth drift for the rest of your life, and they drift back toward their old positions fastest in the first year after treatment. Whatever moved them, the result is held by a retainer: usually a fine wire bonded behind the lower front teeth, plus a clear tray worn at night. The other question is aligners against veneers, since veneers are faster. Aligners move your own teeth and remove nothing. Veneers reshape the teeth you already have, and that enamel does not grow back. If the teeth are sound and the complaint is position, moving them is the smaller intervention, even though it takes months rather than weeks. If the complaint is shape or color, moving them will not fix it.

What clear aligners cost

Clear aligner treatment is quoted as a course, not per tray. Where your case sits depends on how many trays it needs, so we quote after the scan.

The written, itemized estimate comes before anything begins: consultation and records, the scan and plan, the trays, attachments, any interproximal reduction, your reviews and first retainers.

Not included: dentistry needed before we start, such as fillings or gum treatment; whitening; veneers or bonding afterwards; replacement retainers; and lost trays.

Some PPO plans carry an orthodontic benefit, capped per case and sometimes limited by age, and we check yours before you commit. How long clear aligner treatment takes

A typical adult case runs four to twelve months from the first consultation. Small relapse cases confined to the front teeth are often three to six months, while both arches or heavier crowding run beyond a year.

Allow a week or two between the scan and the trays arriving, plus time at the start for fillings or gum treatment; refinements typically add four to twelve weeks. How much do clear aligners cost in Orange County?

Do you do Invisalign?

We provide clear aligner treatment using Candid Pro, which is not Invisalign. Both work on the same principle: a planned series of trays, bonded attachments and light continuous force. Your diagnosis, your plan and your wear time shape the outcome more than the brand does.

Are clear aligners as good as braces?

They are different tools rather than better or worse ones. Braces are fixed to the teeth and work continuously, and they deliver movements trays struggle with, such as bodily root movement and large bite corrections. Aligners come out, which makes wear time your responsibility.

My teeth moved after braces years ago, can aligners fix that?

Usually yes, and often in less time than the original treatment took, because relapse tends to involve small movements of the front teeth. We scan and tell you whether yours is a short case. Retention afterwards is what failed the first time.

How many hours a day do I really have to wear clear aligners?

20 to 22 hours a day, meaning out only for meals and for drinks other than water. That is not padding. Below roughly 20 hours the trays stop seating fully and the case usually has to be rescanned and restarted.

Do clear aligners hurt?

Each new tray feels tight, and teeth are usually tender for a day or two, most noticeably on biting. Most patients manage with over-the-counter pain relief and find it eases as the week goes on. Starting a new tray at bedtime helps.

Will I need a retainer after clear aligners?

Yes, and indefinitely. Teeth drift back towards where they started and adult jaws go on changing, so retention is what holds the result. Most people wear a removable retainer at night, sometimes alongside a thin wire bonded behind the front teeth.

Should I get aligners or veneers?

If your teeth are healthy and the problem is that they sit crooked, aligners are the more conservative route, because they move your own teeth and leave the fronts untouched. Veneers are quicker and change color too, but the enamel they remove does not grow back.
