What happens at a first visit, and how long does it take?

General dentistry is the ongoing care that keeps a mouth working: examinations, x-rays, cleanings, fillings, crowns, extractions and the treatment of pain. Pearl32 Dentistry and Wellness has three general dentists at 24022 Calle De La Plata, Suite 450 in Laguna Hills. Same-day emergency slots are held each day.

Most people reading this are new to Orange County. Or overdue by years and not keen to discuss it. Or leaving an office where something went wrong. This page is for all three.

Book about an hour. Most of it is looking and talking. Nothing is done that you have not agreed to.

  1. Medical history. Conditions, medications, allergies. Blood thinners and bisphosphonates change how we treat you.
  2. X-rays. Usually bitewings. If another office took a set recently, we ask them for it.
  3. The examination. Every tooth, old fillings and crowns, your bite and jaw joint. Gums measured at six points per tooth. Then a soft tissue check of tongue, cheeks and throat.
  4. Photographs, or digital intraoral scanning, so you see what we see.
  5. The conversation. What is there, what needs doing now, what can wait, and what it costs.

Whether you are cleaned that day depends on the gum measurements. Healthy gums, and it usually happens then. Deep pockets and bleeding, and that is a separate treatment.

What are the x-rays for, and what does the cone beam do?

Decay usually starts where a mirror cannot see it: between the teeth, under old fillings. That is what a bitewing x-ray is for, every one to two years for most adults. A periapical shows one tooth and its whole root.

A 3D cone beam CT rebuilds the jaw in three dimensions. A root, a nerve or a bone edge can then be viewed from any angle. It answers what flat images cannot: how much bone an implant has, and where the nerve runs in the lower jaw.

A 3D cone beam CT scan carries more radiation than a bitewing x-ray. So we take one when the answer changes the plan, not as routine. See digital dentistry.

What is the difference between a cleaning and gum treatment?

A routine cleaning and a deep cleaning are not the same appointment at two prices. They treat different conditions.

A routine cleaning is for gums that are healthy or close to it. Plaque and hardened calculus come off the teeth and just below the gum line, in thirty to forty five minutes. It is maintenance, not treatment for disease.

Scaling and root planing, which most people call a deep cleaning, treats gum disease. Root surfaces are cleaned inside the pockets, where a routine cleaning cannot reach. The area is numbed and the mouth is done in two visits. The pockets are measured again six to eight weeks later.

Here is the part worth knowing before you sit in any dental chair. Deep cleaning gets recommended when it is not needed, and skipped when it is. The measurements settle it. Four millimeters and over, with bleeding and bone loss on the x-ray, is gum disease. Bleeding alone, with normal depths, usually is not. So we measure first and read the numbers out to you.

After root planing you do not go back to twice a year. Maintenance every three to four months is what stops the pockets deepening again. Surgery, grafting and relapsing cases are on our periodontics page.

Filling, crown or extraction: how is that decided?

Most restorative work answers one question: how much tooth is left.

A filling replaces what decay or a chip took away, in tooth colored composite, bonded and polished in one visit. Size is what ends a filling. A wide filling leaves thin walls, thin walls flex, and a cusp eventually splits off. Then the tooth needs a crown.

A crown covers the whole tooth and takes over the biting surface. Our in-house CAD/CAM mill cuts it from a ceramic block. A straightforward crown is scanned, designed, cut and bonded in one appointment. No temporary. No second visit. Two to three hours in the chair. A front tooth that has to match its untouched neighbor is often better layered by hand at a laboratory. See same-day crowns.

A tooth comes out when it cannot be rebuilt: a root split lengthwise, decay below the bone, or gum support already gone. We numb the site, and sedation is available. Expect swelling peaking around day two or three, and soreness for several days.

Decide about the gap at the same time, not a year later. Bone shrinks where a tooth used to be, and the first six months take the most. A socket graft at removal holds the shape for a later dental implant.

Seen today: pain that kept you awake, a swollen face or gum, a tooth broken to a sharp edge, or a crown or filling out. A knocked out adult tooth goes back in the socket, or into milk, and needs us inside the hour.

Say so when you book. It changes how much time we set aside and nothing else.

Nobody here is going to comment on the gap. Two things keep people away: expecting a lecture, and expecting a number so large that saying it out loud makes the problem real. Neither is a reason to keep waiting.

The appointment runs as it does for anyone else. We look, we tell you what is there, and we put it in order. Pain and infection first. Then whatever is actively getting worse. Then the rest, at a pace that suits your life.

You can book an examination only, with no treatment that day, and take the findings and an itemized estimate home. Plenty of people start there.

Ten years away is frequently less damage than people have braced for. And if a previous dentist hurt you, tell us what happened and where. That is clinical information, not a complaint about a colleague. We numb thoroughly, we stop when you raise a hand, and sedation is available. Most people are comfortable through routine treatment.

  • Root canals. Straightforward ones are done here. Curved canals, retreatment and odd anatomy go to an endodontist, and we say so rather than struggle.
  • Wisdom teeth. One sitting on the nerve canal goes to an oral surgeon.
  • Young children. Call and ask what ages we are taking. A child needing extensive treatment or sedation does better with a pediatric dentist.

Every six months for most adults and children. Patients with gum disease come every three to four months for periodontal maintenance, because the pockets refill with bacteria faster than that. Most crowns, yes. The tooth is scanned, the crown is designed and milled on the CAD/CAM unit in the office, and it is fitted the same day. Fillings are always one visit.
